Reason for referral
Please include clinical details, management to date, investigation results and any significant medical or social history.
If there is possibility of developmental delay or suspected autism spectrum disorder has been raised, please mention this as will enable us to allocate the time it requires to do a thorough assessment.
If the patient is a current client of Kids First Paediatrics or already known to Dr Geoff Bent and there has been no change in the patient’s condition but referral has expired 1-2 lines of brief history is required.
